Emergency Department Follow-Ups
The Emergency Department (ED) Follow Up program provides follow-up care to patients after they are discharged from a hospital. A formal relationship with the local ED enables uninsured patients to establish Lestonnac as their medical home. This prevents uninsured patients from going to the ED for chronic care management and allows them to receive timely care at Lestonnac. By eliminating the dependence on the EDs for treatment of non-emergent and chronic conditions, patients can better manage their health for improved long term outcomes.
